Registration for the 2019 Recreation Ski Club is scheduled to open on Friday, October 12th at 5 PM and will remain open until Friday, November 2nd. Space in the program is limited and is assigned on a first-come, first-served basis. All necessary forms and information will be posted on this page.
Registration closes on Friday, 11/2 at 4:30 PM. Space for the program is limited and will be assigned on a first-come, first-served basis. Forms not returned by the 11/2 deadline may not be accepted and are subject to a late charge of 20%.
Please carefully review all items listed below before submitting your registration materials. Incomplete or incorrect forms will be returned. Please read all forms very carefully, making sure to sign and initial wherever required.
At a minimum, you must submit the forms contained in links 2 and 3. Please do not send anything directly to Mt. Peter. All items should be mailed or hand delivered to the Department of Recreation at 50 Kings Road.
If you would like to chaperone, please e-mail the Department of Recreation at email@example.com to verify that there's an opening and complete the necessary forms.
All chaperones ski and ride free of charge. All fees are waived for the children of chaperones who are able to attend at least five trips. If you plan on meeting this requirement, simply write "CHILD OF CHAPERONE" on top of your child's registration forms and complete the chaperone forms. This 5-trip minimum can be split between 2 adult family members. Any fees paid to Mt. Peter are non-refundable.
Unless you've already done so for another program, chaperones must complete the online background check for coaches as well as the CDC's online concussion safety course. The online course takes roughly 20 minutes to complete and the background check costs $12.90. The check and course are good across all programs for one full year, unless the program has its own specific requirements.
Bus transportation costs $135 total for all 6 trips. Checks for this portion of the program should be made payable to "Borough of Madison" in the amount of $135.
Please make sure you complete and sign the Code of Conduct portion of the form. Any violations of the Codes of Conduct may result in removal from the program and jeopardize participation in other recreation programs as well. The chaperones and bus drivers are to be treated with the utmost respect at all times. It is very important that we clean up after ourselves at all times.
All forms should be mailed to the Department of Recreation at 50 Kings Road. You can also hand-deliver them to the Borough Clerk's Office in the Hartley Dodge Memorial Building Monday-Friday between the hours of 8:30 AM and 4:30 PM.
Please make sure everything is in order prior to submitting. Incorrect or incomplete forms will be returned.
