HANDICAP PLACARDS
Handicap Placards
Permanent Placards:
Permanent placards are only available if you are permanently handicapped.
- Placards may be used with any vehicle in which the handicapped person is a passenger, regardless of whether that person owns the vehicle.
- If the placard is lost or stolen, it may be replaced by returning the Handicapped Identification Card with an application to the New Jersey State Police. There is no fee.
- Application for permanent placards may be obtained by mail (call 609-292-6500 or toll free in NJ 888-486-3339) or online at www.njmvc.gov.
Temporary Placards:
- Placards may be used with any vehicle in which the handicapped person is a passenger, regardless of whether that person owns the vehicle.
- Temporary placards are only issued to those who have a temporary disability for up to one year only. The placards are valid for six months and may be renewed only once for an additional six months.
How to obtain a temporary handicap placard:
- Pick up the temporary placard application in the Records Dept. of the Madison Police Department.
- Have your doctor sign the application in the doctor certification section of the application.
- Applicant must completely fill out the remainder of the form.
- Return the application, with check or money order made out to “NJ Motor Vehicle Commission” in the amount of $4.00.
- Once the application is returned and completed your placard with be prepared and issued to you.
